Medical Clearance




BEFORE any athletics participation, ALL student athletes are REQUIRED to have:

1. A physical* (from their own physician) medically clearing them to participate in athletics (Only the Athletic Training Physical Form will be accepted)

*Physical MUST be dated within 6 months of initial participation.

2. Proof of insurance (front & back copy of card)

3. Completed and signed Athletic History Form and Information Packet

4.  ALL NEW / FRESHMAN / TRANSFER STUDENT-ATHLETES  are required to confirm Sickle Cell Trait status in all student-athlete pre-participation physical examinations.

July 30 - for the Fall Semester

January 7 - for new Spring Semester students. 

No student will be allowed to participate in any University athletic activity until the Health Record has been accepted.

Once a Health Record is submitted and accepted, this requirement will be met for the student’s entire undergraduate career at Cheyney University.


It is the University’s policy that ALL students show proof that they have met the requirements for current immunizations standards. Questions in this area should be referred to the Health Center at (610) 399-2260. Copies of these records should be provided, along with your physical information to the Office of Athletic Training.

ALERT: In addition to the medical information requested by the Office of Athletic Training, you are required to complete a health information packet for the University Health Center. THIS IS NOT THE SAME PACKET. Make sure you send the appropriate forms to the correct office. Failure to do so may delay your participation and/or registration.

All student-athletes will have a pre-participation medical screening (which is conducted through the Office of Athletic Training before they participate in any University related athletic activity. Examinations will be conducted on assigned dates and times. Students should check with their head coach as to the date, time, and location of their exam. Failure to show for an assigned physical will delay their medical clearance and may result in the student having to pay for an outside physical. Conflicts or other reasonable excuses for inability to attend a scheduled appointment should be communicated to the Office of Athletic Training at (610) 399-2310 BEFORE the scheduled date and time. This exam does NOT take the place of the physical exam necessary for medical clearance.



Personal health insurance plan coverage is required for ALL student-athletes before participation with their sport. Prospective student-athletes MUST have a completed Athletic Insurance Information form and a front & back copy of their insurance card on file with the Office of Athletic Training prior to any participation in team activities. This form is found with the pre-participation medical forms. This form should be submitted to the Office of Athletic Training, with all other forms, by the appropriate deadline. Please complete all requested information in its entirety to avoid any delay in the eligibility process. Each student-athlete is responsible for updating their personal health insurance information should it change. Failure to do so may result in the student incurring unnecessary medical costs should the incorrect procedures be followed. 

ATHLETIC ACCIDENT POLICY (Secondary insurance policy)

Cheyney University purchases an excess basic accident insurance policy, yearly, which covers medical expenses arising from athletic injuries up to a $90,000 limit and in excess of any other available accident/health insurance (such as through the student-athletes primary insurer).

This coverage is paid for by the institution; however, all student-athletes are required to maintain their own primary health insurance coverage to participate. This accident medical insurance coverage applies only to covered athletic injuries and is not a replacement for primary accident/health insurance.

All student-athletes, student trainers, student coaches, student managers, and cheerleaders carrying primary insurance must provide the school with a front and back copy of their card to be kept on file in the athletic training office.

If at any time this coverage expires during the school year, it is the student-athlete’s responsibility to notify the athletic department.

This accident medical insurance policy carries a deductible of $2500. This deductible will be reduced as payments are made by the student-athlete’s primary insurance company. If any portion of the deductible is not satisfied by the primary insurance company this amount will be the responsibility of the student-athlete and/or parent/ guardian.
